Mandatory Uniform & Equipment Fitting

  12-1pm - Pee Wee (Brown) and Jr. Pee Wee (Clemens)

  1-2pm - Mitey Mites (Galoozis & Hoger teams) and Tiny Mites (Patel)

  2-3pm - New Teams

  Players will be fitted for game jerseys, game pants (except Tiny-Mite), and shoulder pads. For those that need to purchase helmets, a representative will also be on-site for helmet fitting. If you have a helmet that you need refurbished, please bring it with you.
   
  If you have not already done so, please bring the following paperwork with you also:
  1) Copy of Birth Certificate.
  2) Parent Consent & Player Contract - don't forget that both the parent and player need to sign this. You can download via the below link.
  3) Doctor's Physical - there are two pages to this form (see below link), one for the parent and one for the doctor. The doctor can use their own form if they wish, but in that case the parent still needs to fill out the Parent page on the Pop Warner form. Also, the physical MUST be performed and dated in 2016.
  4) Final School Report Card - we know you can't do this for another month, but wanted it on your radar.
